What Causes Spider Veins? An Aesthetic Problem or a Health Problem?
Spider veins, the thin, red-purple lines and web-like patterns seen on the legs, are a subject that many of our patients wonder about from both an aesthetic and a health point of view. In this article I will explain the causes of spider veins and what they really mean.
Telangiectasia (Spider Web) Veins
Known medically as telangiectasia, these spider veins form as a result of the widening of very thin vessels just beneath the skin; they usually present an appearance like a spider’s web or fine tree branches. Unlike large varicose veins that cause significant reflux, they are generally not on their own a sign of a serious circulatory problem; however, they can sometimes be a small surface reflection of an underlying venous reflux.
Genetic Factors and the Effect of Standing
The likelihood of developing spider veins increases markedly in people with a family history of varicose veins; in addition, occupations that require standing for long periods, hormonal changes (pregnancy, hormone therapy) and excessive sun exposure also contribute to these fine vessels becoming more visible.
Do Spider Veins Cause Pain?
In most of our patients spider veins are completely painless and cause only aesthetic discomfort; however, in some patients they may be accompanied by mild burning, itching, or a slight feeling of heaviness after standing for a long time. When spider veins are widespread and dense, we recommend an assessment with Doppler ultrasound to rule out an underlying reflux in a larger vein.

Do spider veins progress if they are not treated? They may increase in number over time, but turning into large varicose veins is not always the case; this depends on the underlying venous structure.
Is treatment of spider veins necessary? It is not medically obligatory, but if they cause aesthetic discomfort, treatment options (sclerotherapy, CLaCS) can be considered.
Are spider veins hereditary? There is a strong genetic predisposition; their frequency increases markedly in people with a family history of varicose veins.
